APPLICATION DEADLINE FOR 2025-2026 GRANTS is APRIL 1, 2025
Please review the "District Grant Application Checklist 2025-2026" using the link on the left.
The "Application District 7120 Grant 2025-2026" writable PDF form along with the "District 7120 MOU 2024" and "District 7120 Addendum to Club MOU 2025-2026" forms along with any necessary supporting documentation must be emailed to Patrick Stack at pkroc@outlook.com by April 1, 2025. Contact Pat with any questions. If your application is received by March 15, 2025, the District Grant committee will review the application for format compliance and return it to the club for corrections, if necessary. No application will be accepted after April 1, 2025
Your District Grant Final Report is due four (4) weeks after you complete your project and no later than May 15,2026. Your Club will receive District Funds after your Final Report and supporting documentation are accepted. If you have any problems completing your project by May 15, 2026, please reach out to me as soon as you know there may be an issue.
We have replaced the Adobe writable PDF attachment with an Adobe Sign Webform. When you click on the link, it will automatically open the Final Report Form in your default web browser.
One club member that completed the Grant Management Seminar needs to complete the Final Report form. In addition, I need a separate email with copies of all receipts, canceled checks, any other supporting documentation, and several pictures of your project. Make sure you put your club's name on the subject line of the email. You can send the email to pkroc@outlook.com
When you click on the link, it opens the Final Report Form in your web browser. You may have to click on a Continue button at the bottom of the screen indicating that you accept Adobe terms and conditions. All fields with an asterisk (*) are required fields. After you enter the information in a field, press the tab key to move to the next field. If you do not enter information in a required field, you will see the Next required field in the top right hand of the screen. If you click that message, it will take you to the first missing information field. Note the change to the Finance Section: I am asking for the total amount your Club spent on the project. In the past, the report asked you for your club funds minus your District Award. The total of the income section should equal the total in your expense section. When you to enter your District award amount the form will show you the NET Club funds after your reimbursement.
When all the information is filled in, you will see a message at the bottom or top of the screen with a SUBMIT button. When you press that button, you will receive a pop-up message to reenter your name and email address. When you press the submit key, it will prepare the form for delivery to me.
You will receive one more screen which will inform you that you will receive an email to confirm your email address. After you submit this information you can close the browser tab.
Go to your email and look for an email from Adobe (it may take several minutes). Open it and click on: Confirm my email address. You should then receive a message in a separate tab on your browser indicating your Submission of the Final Report has been verified. I will receive a copy automatically. You will also receive another email verifying that you completed the form, and it will allow you to download a copy for your records.
The report and all the support documentation you send me will be reviewed within 3 business days and confirmation emailed back. If you do not hear from me within 3 business days, please reach out to me.
It is not necessary to mail hard copies any longer. Your club is responsible for maintaining the originals of all documentation for a minimum of five (5) years.
HISTORY: District Grants Awarded
Each year, District 7120 receives "District Designated Funds" (DDF) from The Rotary Foundation of Rotary International. The amount of DDF each year is now 47.5% of our donations to the Annual Fund three years earlier. We are then authorized to use half of our DDF for District Grants. Links to previous Grants awarded in recent years are available in the left column.
